Schiefelbusch Clinic: Requesting Services

To request services from the Schiefelbusch Clinic complete the Intake Questionnaire form and fax or mail it to the Clinic. Someone from the Clinic will contact you to discuss the services you are requesting. Please allow 48-72 hours after receipt of the form for this contact to be made. If you are not contacted, please contact the Clinic at 785-864-4690.

The Clinic is fee for service clinic. We have a sliding scale as well as scholarships for those who need assistance. Scholarships for children are supported with funds from the Kansas Scottish Rite Foundation. Please see the Fee Schedule links below for further information. The Clinic accepts some third party reimbursement. Please discuss your particular need with the Office Manager.

Fees for Audiological services and prices for hearing aids are available by calling the Office Manager (785-864-4690) or speaking to the Clinical Audiologist. The Schiefelbusch Clinic currently has hearing aids from Starkey, Phonak, and Oticon manufacturers available for purchase.
